Family medicine services have ended

While Dr. Constantin retired from family medicine practice on December 16, 2022, she and her team will continue to deliver laser services, cosmetic injectables, medical-grade skincare, as well as select medical services including skin checks, medical Botox, and vaginal regeneration. Finding a doctor

Dr. Constantin’s patients must find a new family physician to continue their care. While access to physicians is limited in the Bow Valley, patients may explore options on the Bow Valley Primacy Care Network website at bowvalleypcn.ca and on the Government of Alberta website albertafindadoctor.ca. Patients may also explore virtual or tele-health options from providers across Canada by searching “virtual doctor Canada” in any search browser.

Appointments and prescriptions

Our clinic is no longer accepting family medicine appointments or prescription renewals. Patients are encouraged to find a new family physician to continue their care. In some cases, patients may be able to renew prescriptions directly with their pharmacy.

Requesting your medical records

Medical records will continue to be available at the clinic. We encourage patients to find a new family doctor before requesting records. To request a copy of your records, please email threesisterslaser@gmail.com. Clinic staff will contact you to confirm the request and arrange payment of the $25 record transfer fee per adult. Records will be provided in digital PDF format only.
